Our Approach
Forever Life Home Health Care is different from other home care agencies because we do the following:
An RN will conduct a "Potential to Fall" assessment on all new patients, ensuring we create a specific plan, and train our staff to prevent you or your family member from falling while we are on your premises.
Our RN will conduct a "Home Safety Check" to ensure your home is clear of anything that can cause you to fall.
Our RN will advocate for other safety equipment if needed.
We also help to manage chronic diseases such as diabetes and high blood pressure using Minnesota Community Standards.
In addition, our RN will train our Personal Care Assistant ( PCA) to help you do your diabetic blood tests and diabetic foot checks.

We provide Personal Care Assistants (PCA's) who help our patients/clients with Activities of Daily Living (ADLs) - grooming, bathing, feeding, mouthcare, light meal prep, laundry, and light housekeeping.
